Therefore, if any cities or nations are destroyed, it's from a much higher level of people. Not just the military, but an overall power structure.
So if the entire world or even just part of it is destroyed, it is as a result of the power structure wanting to take that course of action.

Even if there is never quite a global nuclear war, some people think that there should be a lot more nuclear+power plants constructed. That way even more toxic waste can be created. The problem is, that most people wouldn't benefit from more nuclear waste, any more than from a global nuclear war.
